Twelve questions, two minutes, and what your score means for your golf.

You've played this course for years. Lately the back nine costs more than it used to: a backswing that doesn't go where it used to, a lower back that wants the cart ride home, a jaw you didn't notice was clenched until after the putt. This isn't about your swing. It's about the fascia wrapped around every muscle and joint you use to make that swing, and whether it still moves the way it did.
Fascia is the connective webbing wrapped around every muscle, joint and organ in your body. When it's hydrated and moving freely, force travels through you cleanly: backswing, weight shift, release. When it stiffens, everything downstream gets harder. The backswing shortens. The lower back takes the load it wasn't built to carry. The neck won't rotate as far one way as the other.
The twelve questions below aren't about mechanics or lessons. They're a map of where that tension has settled in. Score yourself as you are now, not as you were five years ago.
One honest line before you start: this will not add distance to your drive. There is no evidence that fascia work does that, and I'm not going to tell you otherwise. What it can do is give tissue that's been held tight for years a chance to move like tissue again.
For each statement, pick a score: 0 (rarely), 1 (sometimes), or 2 (most rounds). Your total updates automatically.

Daily practice loosens surface tension. It won't touch a holding pattern that has built up over years of playing through it, especially if your score landed above 12. That is fascia work done with guided hands, not stretching alone.
A single 45-minute session gives you a fascial assessment and an immediate, felt release. It's not a swing lesson and it doesn't claim to be one. It's a look at what your own tissue is doing, and a start on undoing it.
